Dietitian for Women’s Health is a Controller of the personal data you (the data subject) provide us.

Personal Data: name, address, contact details, date of birth, gender, next of kin

Sensitive Personal Data, with your consent, regarding patient and family medical history, details of medical tests and procedures, medication details, biochemical, haematological and microbiological details, allergy details, anthropometrical details.

 

2. Why We Collect Your Personal Data?

One to One Consultations
We collect your personal data because you give us consent to do so, to provide our independent nutrition and dietetic services to you.

We require this information for:
Professional clinical record keeping of client information in accordance with the Irish Nutrition & Dietetic Institute (INDI) Guidance for Records and Record Keeping.
Sharing information with relevant health care professionals (only when consent is given by the client).

 

When Visiting our Website

When you visit this website, your IP address is anonymised before this is shared with Google Analytics (in order to monitor broad trends). This means that no data is collected which could identify an individual. Certain information is also collected using JavaScript, such as: the browser type and version, time zone setting, operating system and platform, information about your visit, including specific webpages on the site, clickstream to, through and from our site (including date and time); products you viewed or searched for; page response times, download errors, length of visits to certain pages, page interaction information (such as scrolling, clicks, and mouse-overs).

 

When Joining Our Community

If you choose to join our community, your email address is added to a secure list within our Convertkit account in order to allow me to send you information and offers that I feel may be of interest to you. All subscribers to this email list are clearly informed about how their data will be used, stored, processed and tracked using the Convertkit on my community sign up notice before choosing whether to opt-in or not. Subscriber’s email addresses are used for no other purposes than those outlined above, and they are free to opt-out at any time. Verifiable consent is obtained using a General Data Protection Regulation (GDPR) compliant sign-up form, which stores permission data in list export files. A double opt-in method is also used to ensure explicit consent is obtained for the subscription. All subscribers are free to change their information or unsubscribe from our community at any time.

 

When Making a Purchase

Only the information which a visitor enters when making a purchase (first name, last name, company name, address, town, county, Eircode, phone number & email address) is collected on this site. No other information is collected.

Payment information is processed by a 3rd party payment site and is never saved by this website.
As indicated above, this personal user information is obtained when the website visitor has entered their details in order to proceed with the placement of an order. This information is needed by us to process the required order. This personal data is stored in a password protected database. Details are not shared with any third party.

We strive to keep your personal data safe and only share it when necessary. We recognise that you have a right to know that the information you share with Dietitian for Women’s Health is maintained confidentially.

We do not rent or sell your personal data to anyone.

4. How long we keep your personal data?

Under statute, we keep all patient information for eight years. After that period, confidential shredding of paper files takes place and electronic data is deleted. 
Sometimes there may be a circumstance where data is required to be held for a longer period, and this will be outlined to you.

We use the following cookies, but they do not collect personal data:

(A) Necessary cookies: used to serve up the site.

(B) Analytics cookies: to help track site visits and to improve the content of our web pages.
